Harmony Virtual Care Membership Disclaimer

Harmony Virtual Care offers different membership tiers with varying levels of benefits. Please review the following details regarding lab and diagnostic testing:

• Harmony Essential: Lab testing and diagnostic testing are not included in this plan. Members may choose to pay for these services separately at standard rates.
• Harmony Enhanced: Members receive a 50% discount on lab testing and diagnostic services. Pricing details can be obtained by contacting our office.
• Harmony Elite:
       Includes one wellness visit per year at no charge, which covers standard labs.
  • Each in office visit allows one diagnostic test or lab at no charge.
  • If more than one test is required per in office visit, members receive 75% off any additional lab or in-office diagnostic testing.
